How Hormones Affect Your Mental Health

Mental health doesn't exist in isolation from the rest of your body. Hormones — the chemical messengers that regulate nearly every biological process — have a profound effect on how we feel, think, and function day to day.

The Estrogen-Mood Connection

Estrogen plays a significant role in regulating serotonin, dopamine, and norepinephrine — three neurotransmitters central to mood, motivation, and focus. When estrogen levels fluctuate or decline (as they do during the luteal phase of the menstrual cycle, postpartum, and perimenopause), mood can shift dramatically.

This is why many people notice:

  • Premenstrual mood changes — irritability, low mood, or anxiety in the week before their period
  • Postpartum depression — often linked to the dramatic hormonal drop after birth
  • Perimenopausal anxiety and depression — frequently underdiagnosed and undertreated

What makes estrogen especially impactful is its relationship with the serotonin transporter. Higher estrogen levels are associated with more available serotonin at the synapse — which is why many women find their mood most stable in the follicular phase (the week after their period ends) and most fragile in the late luteal phase just before bleeding starts.

Progesterone: The Calming Hormone With a Complicated Side

Progesterone is often described as the calming counterpart to estrogen — and in some women, it genuinely promotes relaxation and sleep, particularly in the mid-luteal phase. But progesterone's metabolite, allopregnanolone, acts on GABA receptors in the brain in a way that some women tolerate poorly. Instead of calming, they experience anxiety, irritability, or depressed mood during the progesterone-dominant phase of the cycle.

This sensitivity to neurosteroid fluctuation is thought to underlie conditions like PMDD and may also explain why some women respond poorly to certain hormonal contraceptives.

Thyroid Function and Mood

An underactive thyroid (hypothyroidism) is strongly associated with depression, fatigue, and cognitive slowing — sometimes called "brain fog." An overactive thyroid (hyperthyroidism) can cause anxiety, racing thoughts, irritability, and insomnia, symptoms easily mistaken for a primary anxiety disorder or even bipolar disorder.

  • Symptoms don't respond as expected to standard treatments
  • Fatigue and cognitive changes are prominent alongside mood symptoms
  • A patient has a personal or family history of thyroid disease

Thyroid dysfunction is common, underdetected, and highly treatable. Ruling it out early prevents years of misdiagnosis.

Testosterone's Often-Overlooked Role

Testosterone — present in all genders — influences energy, confidence, libido, and mood. Low levels are associated with fatigue, depressive symptoms, and reduced motivation in both men and women. In women, testosterone levels decline gradually across adulthood and can drop sharply during perimenopause and after surgical menopause.

Despite this, the connection between testosterone and mental health is rarely discussed in outpatient psychiatric care. Hilda includes testosterone in the hormonal picture when fatigue, low motivation, or sexual health concerns are part of the clinical presentation.

Cortisol, Stress, and the HPA Axis

Chronic psychological stress elevates cortisol — and chronically elevated cortisol has downstream effects on nearly every aspect of mental health. It suppresses sex hormone production, disrupts sleep architecture, impairs memory consolidation, and over time can contribute to depression and burnout.

This is why stress management isn't just a lifestyle recommendation — it's a biological intervention. Reducing chronic cortisol burden can support more stable mood, better sleep, and a more responsive hormonal system.

What a Hormone-Informed Evaluation Looks Like

Rather than treating mood symptoms in isolation, Hilda considers:

  • Symptom timing relative to the menstrual cycle — are symptoms worse in the luteal phase? Post-ovulation? In the days before bleeding?
  • Reproductive life stage — postpartum, perimenopause, post-menopause, after recent hormonal contraceptive changes
  • Thyroid and metabolic history — including lab trends over time, not just single-point values
  • Sleep, energy, libido, and cognitive changes alongside mood symptoms

This doesn't mean every mental health issue has a hormonal cause — but dismissing hormonal context means treating symptoms without understanding their source.

  • Your mood symptoms follow a predictable monthly pattern
